Highly professional patient-focused GP care from our experienced and compassionate doctors. Residents receive a Comprehensive Medical Assessment on admission, which is reviewed annually. Residents with acute presentation are prioritised and medical reviews are delivered promptly. Our GPs are available for once-off visits, regular scheduled visits, chronic disease management, direct care conferences, and multi-disciplinary team meetings.
Ideal Care nursing staff do not aim to replace Aged Care facility nurses, but support them in ways that are best for the resident. Prescribing indicated non-pharmacological and pharmacological interventions, our nurse practitioners specialise in nursing care and chronic disease management. Practitioners and registered nurses are available for regular visits, care plan reviews, education and support, liaising with facility nursing staff, as well as facilitating case conferences and family meetings.
Our liaison personnel exist to build relationships through open communication, keeping all parties aligned and on the same page. In fact, we believe it’s our communication that brings the biggest comfort to residents and their families. We aspire to strengthen partnerships between our staff, families, Aged Care facilities, and external providers. Being highly organised, our expert communicators facilitate regular visits and periodic scheduled meetings with service providers, as well as developing identified training needs for Ideal Care personnel.

That’s why Ideal Care encourage specialist allied health services when necessary, which we facilitate by referral. Operating under a holistic approach, this includes all areas of health and wellness, such as mental health, occupational therapy, optometry, physiotherapy, podiatry, speech therapy, dental screening, and more.
